I have made a new template document with the following page format: A4, portrait, different first page, 2 cm top margin, 2cm bottom margin, 3,17 cm left margin and 1,5 cm right margin. I have added a five row two column table in the header area of the second page. The two cells in the bottom row of the table have been joined together (to hold the document title). The left upper four cells have also been joined together to form a large area that contains a picture frame in the left half of the area and a text frame in the right half of the area. The picture frame contains a company logo (logo.ico) and the text frame contains six lines of text (company address). There are more pages in the template that contain boilerplate text. The table appears in the header area of these pages but there is no picture frame and no text frame in the table (and thus no logo and no address)

Is there a particular way to include picture and text frames in the header area so that they appear on subsequent pages?

Headers can be a bit finicky when it comes to frames. I recommend to leave the header and the table out and to put simply one picture frame (upper left) and two text frames (upper right and below) in object mode on the master page (View :arrow2: Master pages). If you like, you can group them for additional security, but that's not really necessary.
